The North Florida/South Georgia VA Healthcare System (NF/SG VAHS) is recruiting a fulltime Advanced Practice Registered Nurse to work within the Home Care/Home Based Primary Care (HBPC) Program with Geriatrics and Extended Care service. The APRN Nurse Educator provides for the assessment, planning, organization, implementation and evaluation of an effective staff development program including orientation, continuing education, competency development and activities that support/improve the delivery of healthcare in the home based primary care setting and the organization. The Nurse Educator establishes effective relationships with all Nursing Service personnel, and serves as liaison between the medical center, affiliated agencies and the community. The Nurse Educator is a role model in demonstrating excellence in practice, serves as a clinical mentor/preceptor for clinical/administrative staff, promotes specialty certification through education, provides formal/informal training, provides clinical supervision of staff members/students, and consults/collaborates in patient care and research. The Nurse Educator collaborates with the home-based primary care leadership team to support evidence-based research initiatives, oversee quality improvement, maintain safety and quality standards as well as support strategic initiatives. Additionally, the Nurse Educator participates in patient care delivery, as needed, for patient care and education and may provide some primary care services at home to a clinically and psychosocially complex population of chronically ill or severely disabled Veterans of the NF/SG VAHS, and may support the provision of care by others to the Home Care/Home Based Primary Care population. Key responsibilities include: The Nurse Educator will directly foster HBPC PACT patient education through technology geared towards Veterans and caregivers, and indirectly through clinical technological modality education and support of HBPC PACT members. The Nurse Educator will maintain nursing competency folders. The Nurse Educator will work directly with Homecare Performance Improvement Liaison to ensure all nursing staff are educated/ re-educated regarding quality metrics in our system. Other duties as assigned by the Program Director/Chief, Home & Community Based Care, and the Associate Chief of Staff, Geriatrics & Extended Care. Work Schedule: Monday - Friday, 7:30 a.m. - 4:00 p.m. or 8:00 a.m. - 4:30 p.m. Financial Discloser Report: Not required
