This position is located in the Primary Care service of the Stratton VA Medical Center. You will function as a integral member of the Patient-Aligned Care Team and will be expected to work closely with team members, including nursing and administrative staff, in the coordination of patient care. You will participate in daily huddles with team members and in primary care systems redesign projects. You will be assigned a panel of patients to manage under the Department of Affairs Primary Care Management Model guidelines. recommended panel size at the division level is calculated based on patient complexity, support staff, and room availability. You will be expected to provide clinical oversight and participate in peer review of other primary care providers through periodic chart reviews. Your basic responsibilities will include: Providing patient care. Acting as surrogate for absent provides when needed. Attending staff meeting and medical meetings. Collaborating with and supervising mid-level providers. Following Veterans Health Administration guidelines. Timely documentation of encounters. Working closely with Patient-Aligned Care Team (PACT) including nursing and administrative staff. Working within your scope of practice.
