Position Summary: We have an exciting opportunity to join our team as a Clinical Coord.-BeInto Health.
In this role, the successful candidate Complete biannual screenings, care planning, case conferencing, and health education to support comprehensive care and case management for eligible clients enrolled in the BE InTo Health grant program (individuals living with HIV who are over age 50 and are Black and/or Latino).
Complete and document biannual screenings for cognition impairment, depression, activities of daily life, falls, and medication review (i.e., MOCA, PHQ-9, ADLs, etc.) for all enrolled patients. Ensure all patient screenings are completed and up to date according to the screening schedule in the project work plan.
Develop a client-centered plan in response to the initial intake and geriatric screenings, listing clients goals for participation in the program, the actions that both the client and program staff will take to achieve goals, program services that will be delivered and service frequency.
Review care plans regularly to evaluate progress toward achieving goals and make revisions in response to the reassessment or emerging needs.
Hold case conferences with primary care provider or HIV specialist to jointly assess and evaluate client, coordinate services and assess progress toward care/service plan goals. Case conferences may also include patient and other service providers such as patient navigators, pharmacists, behavioral health providers and other members of the care team.
Plan and lead health education sessions with enrolled patients; both individually and in groups.
Confirm enrolled clients are provided Hepatitis C screening and treatment, if applicable.
Oversees clinical quality management efforts regarding services provided to enrolled patients, including providing recommendations regarding quality measures and QI projects.
Provide input into overall program structure and additional social and health education needs of patients in order to ensure optimal programming.
Minimum Qualifications: To qualify you must have: a MSN, medical degree, or equivalent; experience with care coordination and coordination of supportive services (RN with masters degree or extensive relevant experience may be considered)
Preferred Qualifications: Preferred: At least 2 years of experience caring for LGBTQIA+ people with HIV
