The Restorative Nurse provides direct and indirect nursing care to residents and families in the Community Living Center (CLC). The incumbent is accountable for adhering to established policies, standards of care and standards of practice. This Staff Nurse demonstrates leadership when providing and improving resident care. The CLC Restorative Nurse reports directly to the CLC Nurse Manager. The Restorative Nurse serves as a role model when functioning as a team leader or charge nurse under the supervision of the nurse manager or designee. The Restorative Nurse demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The unit/area specific scope of practice should include the following: Area of Practice (Community Living Center) Resident Population (elderly, residents with functional decline, short term rehabilitation) Unit/Position specific competencies. Major duties and responsibilities include, but are not limited to: Provides a positive and supportive relationship with the Veterans who have a restorative nursing order. Acknowledges the Veteran's limitations while maximizing individual participation and achievement in exercise(s). Utilizes the nursing process and accurately documents care of Veterans in their medical record. Familiars with preventing or minimizing disabilities. Maximizes cognitive and physical abilities while maintaining highest possible functional levels. Fosters open communication with health care team members, residents, families, and promptly works to resolve their issues and concerns. Identifies restorative needs and initiates early interventions. Collaborates with Physical Therapy (PT) to ensure there is an order in Veteran's chart for Restorative Nursing prior to discharge from PT. Demonstrates strong customer service skills. Provides education for residents and families. Identifies quality improvement activities and leads any restorative quality improvement processes. Provides training and guidance to nursing staff, Veterans, and the interdisciplinary team to ensure that the most current evidence-based practice of Restorative Nursing is being utilized. Works with the delegated CLC Physical Therapist and Occupational Therapist to ensure nursing staff are trained and proficient in the safe use of assistive devices to promote Veteran self-care and mobility. Ensures Restorative Therapy documentation in the medical record is being completed timely and appropriately by CLC nursing and communicates any concerns to the CLC Nurse Manager and/or Medical Team.
