SUMMARY OF POSITION:
Primary responsibility is to manage care for patients with Multiple Sclerosis (MS) to promote effective education, self-management support, and timely health care delivery. This will include developing and monitoring care management processes that support the MS clinical teams.
Bachelor's Degree in Nursing required.
Currently licensed to practice as a Registered Nurse by Pennsylvania Board of Nurse Examiners required.
Three (3) years of nursing experience required.
KNOWLEDGE, SKILLS, & ABILITIES:
Experience in coordination of patient care involving an interdisplinary team of physicians, therapist and social worker
Master's degree preferred.
Prior experience with MS preferred.
