Christiana Care is looking to hire a full time School-Based Health Center Project Coordinator in Wilmington, DE.
To support and coordinate the School-Based Health Centers (SBHC) operations. Participate in the expansion and strategic planning for SBHCs and opening of new centers. Assist Lead SBHC Program Manager with SBHC operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Identifies service needs, program/service development, and implementation and evaluation of services and integrates SBHCs into ChristianaCare (CC) operations
Supports the existing SBHCs to ensure uninterrupted workflow and continued revenue collection
Participates in the expansion, planning and opening of new SBHCs
Assists the SBHC Lead Program Manager with the proposal and contract preparation process along with submissions
Coordinates new site startup to include hiring and training staff, site evaluation and setup, oversight of supply ordering, obtaining required certificates, liaison with school, state and ChristianaCare departments to satisfy project needs
Assists Lead SBHC Program Manager with reporting requirements for existing SBHCs
Assists SBHC Program Manager with fiscal management including timekeeping and proration process for SBHC employees
Reviews SBHC timekeeping on a bi-weekly basis with SBHC Staff Assistant
Provides oversight of SBHC Staff Assistants and Administrative Assistants (AAs) at SBHCs
Coordinates the hiring and orienting of new AAs with SBHC Staff Assistants
Reviews department processes frequently and update as needed to assure that the SBHCs deliver a high level of patient-centered care.
Works collaboratively with other Medical Group practices, departments and community partners to provide excellent patient care.
Attends regularly scheduled staff meetings, team or center wide
Represents SBHCs at CC and other appropriate meetings when requested.
Heightens the awareness of SBHCs within CC and the community.
Maintains up to date knowledge of CC and departmental policies and procedures, providing staff with pertinent information as needed.
Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to manager, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Performs other related duties as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Bachelor's degree required. Health program experience may be substituted in lieu of a bachelor's degree.
5 years of healthcare related experience required, preferably in school-based healthcare.
3 years of supervisory experience highly preferred.
3 years of community health experience preferred.
KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:
Knowledge of public health principles, theories and practices.
Strong working knowledge of community resources, state and federal programs.
Ability to perform administrative skills.
Ability to work with minimal direction and supervision.
Ability to work in dynamic, constantly changing environment.
Ability to perform multi-tasks daily.
Ability to work with wide range of clients with varying needs.
Strong verbal and written communications skills.
Skills in analytical thinking.
Strong working knowledge of Word, Excel and RedCap.
