The Sioux Falls VA is recruiting for a Home Based Primary Care (HBPC) Case Manager (RN). Come join our team!! The Home Based Primary Care (HBPC) RN Case Manager performs appropriate assessment of the patient or situation by using established case management processes and standards involving the baseline and ongoing level of physical, emotional, psychological, and spiritual functioning and looking at all the resources available and the use/need for them. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
